29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:      (a) On or about September 29th, 2014, at a job site located at 5414 Baltimore in Kansas City, MO, employees were performing excavation work and were exposed to trench collapse and other related hazards.  The employer had not developed or implemented accident prevention programs and had not provided any training related to such programs.      Abatement certification is required for this Item.  Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.    NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. 29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:       (a) On or about September 29th, 2014, at a job site located at 5414 Baltimore in Kansas City, MO, employees were exposed to possible head injuries while working around a backhoe and while working in a deep unprotected  trench.  The employees were not wearing hard hats.       Abatement certification is required for this Item.  Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.     NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. 29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:      (a) On or about September 29, 2014, at a job site located at 5414 Baltimore in Kansas City, MO, employees were exposed to potential trench collapse and engulfment hazards while working in a trench over 12 feet in depth with vertical walls and no safe means of egress.  Employees were getting in and out of the trench by riding the bucket of the backhoe.      Abatement certification is required for this Item.  Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.    NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. 29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). The employer had not complied with the provisions of 29 CFR 1926.652(b)(1)(i) in that:      (a) On or about September 29, 2014, at a job site located at 5414 Baltimore in Kansas City, MO, employees were exposed to serious trench collapse and engulfment hazards while working in a trench that was 12 to 13 feet in depth and 3 feet wide at the top with vertical trench walls.  The employees were not provided with any type of system to protect them from being caught in a trench collapse.      Abatement certification is required for this Item.  Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.     NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.
