29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were not standing firmly on the floor of the basket:  Along the west side of the construction building, on or about October 03, 2014, employees were exposed to fall hazards while standing on the midrail of the basket of a JLG aerial lift model 600S approximately 20 feet high doing painting work.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  Along the west side of the construction building, on or about October 03, 2014, employees were exposed to fall hazards while working from a JLG aerial lift model 600S without wearing a body harness with lanyard at an approximate height of 30 feet to the ground below.
