29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s).  On or about October 2,  2014, the employer did not ensure frequent inspections were conducted and providing a workplace where any employee, worker, laborer or mechanic employed in the performance of the contract were free from recognized hazards such as but not limited to:  a) workers performing decking activities on the roof of a 3 story apartment building without fall protection and exposed to an unprotected fall hazard approximately 35 feet to the ground below; and  b) workers using pneumatic nail guns to install roofing material were not wearing eye protection and exposed to eye injuries.
