29 CFR 1910.22(a)(2)  The floor of every workroom was not maintained in a clean and dry condition:    On or about October 2, 2014, all floors were not maintained in a clean and sanitary condition including, but not limited to:      a.  The floor at the oil pouring station in plant 1 was observed to have excessive residue present that could lead slip and fall hazards.    b.  The floor in the color plant oil room was observed to have excessive oils and other product on the floor, which could lead to slip and fall hazards.

29 CFR 1910.303(g)(1):  Sufficient access and working space was not provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment:  On or about October 2, 2014, not all electrical panels had sufficient access and working space available such as, but not limited to the following:  a.  The electrical panels and shut off switches in plant 1, area #4 were blocked with carts and debris and did not provide adequate working space.  b.  The electrical panels and shut off switches in the maintenance shop, north wall were blocked by carts, boxes, and other materials and did not provide adequate working space.
