29 CFR 1910.67(c)(2)(v):  A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:  Van Ladder aerial lift, Brinks Manufacturing Company, Model # 3924-TGH, Serial No. 10251.  On or about October 2, 2014, and at times prior thereto, employee(s) were exposed to a fall of greater than 10 feet to the ground level below, while cleaning a sign on a building from the basket of an aerial lift, without fall protection.
