29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:  On or about October 6, 2014, at the above addressed site, an employee was exposed to a fall hazard while removing fronds from the bucket of a truck mounted aerial lift approximately 20 feet above a lower level and not being protected from falling to that lower level.

29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects:  On or about October 6, 2014, at the above addressed site, an employee was exposed to potential head injuries when removing fronds of a palm tree above his head while not wearing a protective helmet.
