29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a.   On or about Oct. 7, 2014, on the south side of the house, employees working on a jobsite installing framing material were using pneumatic nail guns and did not wear eye protection, exposing themselves to the hazard of being struck in the eye or face by flying debris.     b.   On or about Oct. 7, 2014, on the south side of the house, an employee working on a jobsite installing framing material was using an electric reciprocating saw and did not wear eye protection, exposing themselves to the hazard of being struck in the eye or face by flying debris.

On or about Oct. 7, 2014, on the southeast side of the house, an employee installing framing was using 2 inch by 10 inch wood planks as a scaffold platform, exposing the employee to an 6 feet, 7 inch fall hazard.  b.  On or about Oct. 7, 2014, on the southwest side of the house, an employee installing framing was using 2 inch by 10 inch wood planks as a scaffold platform, exposing the employee to an 6 feet 1 inch fall hazard.

On or about Oct. 7, 2014, on the southeast side of the new construction building, the carpenter's scaffold did not have adequate firm foundations, exposing employees to a 6 feet 7 inch fall hazard.  b.  On or about Oct. 7, 2014, on the southwest side of the new construction building, the carpenter's scaffold did not have adequate firm foundations, exposing employees to a 6 feet 1 inch fall hazard.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a.  On or about Oct. 7, 2014, on the south side of the building, an employee stepped outside a window opening while building a frame structure for a structure was not protected from a 16 feet 3 inch fall hazard by the use of a personal fall protection system.  b.  On or about Oct. 7, 2014, on the south side of the building, an employee working on an edge while building a frame structure for a structure was not protected from a 11 feet 5 inch fall hazard by the use of a personal fall protection system.
