29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:  Work site, East side: On or about October 7, 2014, employee(s) were exposed to a fall hazard of greater than 10 feet to the ground below from the use of a field designed personnel platform, which was elevated by a JLG Telehandler (Model: G12-55A, Rental ID: 495503) rough-terrain forklift, while transferring stucco to an adjacent scaffold system.

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:  Work site, East side: On or about October 7, 2014, employee(s) were exposed to a fall hazard of greater than 10 feet to the ground below, and struck-by hazards from the use of a field designed personnel platform, which was elevated by a JLG Telehandler (Model: G12-55A, Rental ID: 495503) rough-terrain forklift, and was not secured to the forks or mast.

29 CFR 1926.454(c)(3): The employer failed to retrain employees that showed indications through work performance of not retaining the requisite proficiency:  Work site, East side:  On or about October 7, 2014, and times prior there to, the employer did not retrain employee(s) on the skills or understanding needed for safe work involving the use of scaffolds, where employees entered and exited adjacent scaffold platforms from raised, forklift-supported personnel platforms by climbing over scaffold guardrails, exposing employee(s) to a fall of approximately 50 feet the ground below.
