29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:     Shipping, Clark electric order picker powered industrial vehicle: on or about October 8, 2014, an employee retrieving parts from inventory racking system was exposed to a fall hazard of approximately 14 feet from the highest level where adequate fall protection system was not provided.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:  Shipping, Clark electric stand-up and Yale propane-gas powered sit-down forklifts: on or about October 8, 2014, employees operating forklifts for handling materials and picking products did not receive required operators' performance evaluations as specified in the standard at least once every three years to ensure compliance in safe operation.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
