29 CFR 1910.1030(d)(2)(i): Engineering and work practice controls were not used to eliminate or minimize employees exposure: a) Wahl Family Dentistry: The employers (Dentists) administered subcutaneous injections with a Henry Shine 1 3/8 nerve blocking syringe (25 gauge) which was not equipped with engineering controls. The employer did not ensure that all syringes used at the site were equipped with engineering controls where feasible. This was observed on October 9, 2014.

The employees were not offered the Hepatitis B vaccination series during their employment. This was observed on October 9, 2014.

29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use. a) Wahl Family Dentistry - The employer did not have an adequate eye wash station available for his employees who used the hazardous chemical product MaxiSpray Plus which contains the component 2-Butoxyethanol. This hazardous product which has a PH level between 11.0 and 12.5 is used to disinfect the work surface within the patient areas. This was observed on October 9, 2014
