29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.    A. Carlos Donis DBA Donis Construction worksite located at 3450 Seaman Rd., Oregon, Ohio: On or about October 8, 2014, the employer did not ensure employees were protected from head injuries while working below employees installing siding. Due to this condition, employees were potentially exposed to impact injuries caused from falling material and/or equipment.

29 CFR 1926.501(b)(1) Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   A. Carlos Donis DBA Donis Construction worksite located at 3450 Seaman Rd., Oregon, Ohio: On or about October 8, 2014, the employer did not ensure employees were protected from fall hazards while performing siding activities. The exposed employee was installing siding while working on the low slope overhang exposing the employee to a fall distance of approximately 9.00 feet on the North side of the commercial structure.
