29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports: This violation occurred on October 8 2014, at a jobsite located at 3826 Pinetree Ct. Dallas, TX 75206; where employees working from the third tier of a tubular welded frame scaffold that was not fully decked were exposed to a fall hazard of approximately 18 feet to the concrete below.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access: This violation occurred on October 8 2014, at a jobsite located at 3826 Pinetree Ct. Dallas, TX 75206; where employees climbed 24-feet on crossbraces from the lower surface level to access the third and fourth tiers of a tubular welded scaffold. The employer did not provide a ladder or other means of direct access to the third and fourth tiers of the scaffold.
