29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, personal fall arrest system, or other approved alternative fall protection system:    (a) Guerrero Mendoza at 7926 E 35th Ave, Denver, CO 80238:  On or before October 9, 2014 the exposing employer did not ensure that employees were protected from injury, in that three employees removing and installing shingles on the second level of a two-story residential home were not utilizing a fall protection system.  The pitch of the roof was approximately a 9-in-12.  This condition exposed employees to a fall of approximately 18 feet.        (b) Guerrero Mendoza at 7926 E 35th Ave, Denver, CO 80238:  On or before October 9, 2014 the exposing employer did not ensure that employees were protected from injury, in that one employee removing and installing shingles on the first level of a two-story residential home was not utilizing a fall protection system.  The pitch of the roof was approximately a 6-in-12.  This condition exposed the employee to a fall of approximately 10 feet.
