29 CFR 1926.350(a)(6): Compressed gas cylinders were being moved without being secured on a special carrier intended for this purpose and did not have the regulators removed and valve protection caps put in place:      On or about October 7, 2014 at the jobsite employees were exposed to struck by hazards when an acetylene compressed gas cylinder was transported lying horizontally in an equipment cart that was not designed for this purposed.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    On or about October 7, 2014 at the jobsite employees were exposed to fall hazards greater than than six feet above the next lower level when the employer failed to ensure fall protection was utilized.
