29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables used for temporary wiring were not protected from damage:      a)  Tactical Equipment Maintenance Facility North;  Maintenance Bay;   On or about October 8, 2014, a Genie lift #G27304 was parked on a blue SJTW#12 extension cord which was supplying power to another Genie lift receptacle. This receptacle provided power to a compressor for a spray painting gun which was used to spray primer on overhead beams, HVAC system vents and pipes.

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers.    a)  Tactical Equipment Maintenance Facility North; Maintenance Bay;  On or about October 8, 2014, a concrete maintenance pit measuring approximately 5 feet 7 inches deep by 42 ¼ inches wide by 40 feet long was not guarded by covers.
