29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a)  Darren Blanchard DBA Home Improvements of Michigan on or about October 10, 2014, at the address of 5800 Monroe St. Toledo, OH: The employer failed to ensure employees were protected from falls while both removing old and installing new roofing material on a commercial type structure.  Employees working in this condition were exposed to a 9 ft. fall to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a)  Darren Blanchard DBA Home Improvements of Michigan on or about October 10, 2014, at the address of 5800 Monroe St. Toledo, OH: The employer did not ensure that employees had been trained in the appropriate use of fall protection such as but not limited to the use of personal fall arrest systems or alternative measures while performing roofing operations from elevated surfaces.
