29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: a) Jobsite, 651 Kelso Road, Pittsburgh, PA: On or about October 10, 2014, an employee was putting felt paper on a residential roof and was not protected from falling. The fall hazard to the employee was approximately 17 feet to the ground. The roof pitch was 4 in 12.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a) Jobsite, 651 Kelso Road, Pittsburgh, PA: On or about October 10, 2014, an employee was working on a residential roof without fall protection and had not be trained to recognize the dangers of falling and how to protect against them. The fall hazard to the employee was approximately 17 feet to the ground.
