29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or warning line systems.  On or about October 9, 2014, employees performing roofing activities on the roof of a new building were not provided fall protection exposing the employees to an unprotected fall hazard of more than 6 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  On or about October 9, 2014, employees performing roofing activities accessed the roof area with an extension ladder that the hand rails were not extended at least 3 feet above the upper landing surface exposing employees to approximately a 15 foot fall hazard.
