29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables used for temporary wiring were not protected from damage:  a)      Tactical Equipment Maintenance Facility North; Maintenance Bay;  On or about October 8, 2014, a Genie lift #G27304 was parked on an orange and black SJTW #10 extension cord. The extension cord provided power to a Graco GH 230 convertible hydraulic sprayer which was being used to spray liquid emulsion air vapor/barrier membrant on a rooms walls and ceilings.

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers.    a)  Tactical Equipment Maintenance Facility North;  Maintenance Bay;  On or about October 8, 2014, a concrete maintenance pit measuring approximately 5 feet 7 inches deep by 42 ¼ inches wide by 40 feet long was not guarded.
