29 CFR 1910.177(g)(6): Tires on single piece rim wheels were not contained within a restraining device, positioned behind a barrier or bolted on the vehicle with the lug nuts fully tightened during tire inflation: On or about October 9, 2014, at the front entrance of the maintenance bay building, an employee used a air hose to inflate a trailer tire without using a tire cage.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: On or about October 9, 2014, at this facility, at the front of the maintenance bay, employees used the tire changer on a daily basis and strain relief was not provided to the male end of the cord and plug which was connected to the outlet.
