OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards from falling stock: a) At West Rear Wall of Facility: On or about October 9, 2014 and at times prior; the employer exposed her employees to struck by hazards from falling product and sections or rack in that the employer failed to ensure that column base plates of storage racks were anchored to the floor with anchor bolts. b) At Middle Rear of Facility: On or about October 9, 2014 and at times prior; the employer exposed her employees to struck by hazards from falling product and sections or rack in that the employer failed to ensure that column base plates of storage racks were anchored to the floor with anchor bolts. c) At East Side Wall of Facility: On or about October 9, 2014 and at times prior; the employer exposed her employees to struck by hazards from falling product and sections or rack in that the employer failed to ensure that column base plates of storage racks were anchored to the floor with anchor bolts. d) At North Wall of Facility: On or about October 9, 2014 and at times prior; the employer exposed her employees to struck by hazards from falling product and sections or rack in that the employer failed to ensure that column base plates of storage racks were anchored to the floor with anchor bolts.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crush hazards: a) At Unbelievable Flooring and Decor Montgomery, Alabama: On or about October 9, 2014 and at times prior; the employer failed to require and enforce the use of seat belts for all employees operating forklifts on site exposing them to struck-by and crush hazards from being thrown from the lift.
