29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:  a)  Battalion HQ Building D;  North side of building; On or about October 7, 2014, a blue extension cord was plugged into an extension cord with outlets which powered an Emerson microwave oven.  The UL listing and labeling states that extension cords are not to be plugged into one another or daisy chained.  b)  Battalion HQ Building D;  North side of building; On or about October 7, 2014, a blue extension cord was plugged into an extension cord with outlets which powered a portable light fixture. The UL listing and labeling states that extension cords are not to be plugged into one another or daisy chained. c)  Battalion HQ Building D;  North side of building; On or about October 7, 2014, a blue extension cord was plugged into an extension cord with outlets and another extension cord was plugged into the extension cord with outlets, which powered a portable light fixture in the rear of the trailer.  The UL listing and labeling states that extension cords are not to be plugged into one another or daisy chained.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables used for temporary wiring were not protected from damage:  a)  Battalion HQ Building D;  North side of building; On or about October 7, 2014, a blue extension cord was run from the temporary power pole through the roadway to a job trailer.  The blue extension cord was covered with a green water hose, but it was not protected from vehicular and powered industrial lift crushing hazards.
