29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  On or about October 14, 2014 the employer did not ensure that employees were protected from falling due to not providing any means to prevent employees from falling at the job site exposing employees to fall hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  On or about October 14, 2014 the employer did not ensure that a portable ladder that was used to gain access to the roof was extended at least three feet so as to assist employees in the prevention of falling exposing employees to fall hazards.
