OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970: the employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees where they were exposed to struck-by and amputation hazards.      On or about September 23, 2014, at a location near or at 1201 4550 Pump Station Rd., Jacksboro, Texas: the employer did not ensure continuous visual or alternative communications were used or in place where employees were performing drilling operations exposing employees to struck-by and or amputation hazards.
