OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing and struck-by hazards associated with improper automobile lift operations:     a) On or about October 14, 2014 and times prior, employees were exposed to crushing and struck-by hazards while working around auto lift number five which was not functioning properly and was used daily by the employees.  Three lifting arm slide stops were not installed allowing the lifting arms to slide beyond their safety limit and four lifting arm swing gears were worn out allowing the lifting arms to swing beyond their safety limit. 29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  a)  On or about October 14, 2014 and times prior, the employer did not establish a written respiratory protection program and provide effective training to employees who used respirators to protect themselves from respirable dust and methyl ethyl ketone.  Employees used elastomeric half-face respirators and powered air-purifying respirators (PAPR) without being provided medical evaluations and basic training which ensured that the use of the respirators themselves did not present a health hazard to the employees.    Abatement Note:  Fit testing shall follow the requirements listed in 29 CFR 1910.134(f).
