29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   On or about October 08, 2014,  employees performing work on an overhead canopy were not provided fall protection.  The employees were exposed to a fall hazard of 10 feet to the ground below.

On or about October 08, 2014, employees were using an aluminum extension ladder with side rails that did not extend at least 3 feet above the edge of the landing.  The employees were exposed to a fall hazard.
