29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toe-boards, safety net systems, or personal fall arrest systems:  On or about October 10, 2014, employees installing roofing materials were not provided fall protection and exposed to a fall hazard of approximately 12 feet to the concrete below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about October 10, 2014, the side rails of the ladder used to access the roof did not extend at least 3 feet above the roof eave, exposing employees to fall hazards while accessing and egressing the roof.
