29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:     On or about October 14, 2014, and at times prior thereto, employees used a 20-foot extension ladder positioned on the backside of the house to access the roof, the ladder side rails did not extend at least 3 feet (.9 m) above the roof surface. Employees were exposed to a fall hazard of greater than six feet.

29 CFR 1926.1053(b)(5)(i): The non-self-supporting ladder was not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support):    On or about October 14, 2014, and at times prior thereto, employees used a 20-foot extension ladder positioned on the backside of the house to access the roof, the ladder was not set correctly nor was it positioned vertically at a 4:1 angle. Employees were exposed to a fall hazard of greater than six feet.
