29 CFR 1910.272(e)(1)(ii): There were no specific procedures and safety practices applicable to their job tasks including but not limited to, cleaning procedures for grinding equipment, clearing procedures for choked legs, housekeeping procedures, hot work procedures, preventive maintenance procedures and lock-out/tag-out procedures:    5005 Dakota Ave., South Sioux City, NE 68776 - The employer is failing to ensure that specific lockout practices or procedures were developed to ensure communication between employees who are working together but located in different areas when cleaning a hopper. An employee was injured when their hand was caught in a piece of machinery while assisting two other employees trying to clear a hopper. One of the employees was on a different level operating a control panel and the equipment was not locked out.    Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed.
