29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets. a) On or about October 15, 2014, at the workplace, the employer did not ensure/enforce that employee(s) used hard hat protection while working below employee(s) working above their head, thereby exposing employee(s) to a potential struck-by hazard.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) On or about October 15, 2014, at the worksite, the employer did not ensure/enforce that employee(s) used fall protection while performing roofing work, on the North side ledge of the roof, at heights greater than six feet, thereby exposing employee(s) to an approximate 20 foot fall hazard.
