29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     On the jobsite located at 2578 Pratt Street, Omaha, NE: The employer is failing to ensure that employees engaged in residential construction  were protected from fall hazards greater than 6 feet. On or about October 15, 2014 employees were observed installing roofing materials exposed to fall distances of approximately 12.7 feet and were not protected from the fall by properly installed and/or used conventional fall protection.

On October 15, 2014 an employee was observed using a Werner fiberglass extension ladder placed on top of a loaded roll off container at angle greater than four to one and not secured to prevent displacement.
