29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment shall not be used to support scaffold platforms unless they have been specifically designed by the manufacturer for such use. (a) 2nd floor window ledge - On or about October 14, 2014 two employees were exposed to a fall hazard while standing in an unauthorized open wooden box used as a scaffold. This box was elevated by a Skytrak 6042 Telescopic Forklift serial no. 10147236. This box was not designed by the manufacture to support employees.

29 CFR 1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews. (a) 2nd floor window ledge - On or about October 14, 2014 two employees were exposed to a fall hazard while standing in an unauthorized open wooden box used as a scaffold. This box was elevated by a Skytrak 6042 Telescopic Forklift serial no. 10147236. These employees were elevated 21 feet above concrete with no fall protection.
