On or about November 25, 2014, the employer entered the employee's name on the OSHA 300 Form or equivalent that resulted in a privacy concern case.  a) On or about April 10, 2013, a recordable injury occurred when an employee received a needlestick injury from a contaminated sharps and the employer did not enter "privacy case" in the space normally used for the employee's name.  b) On or about April 16, 2012, a recordable injury occurred when an employee received a needlestick injury from a contaminated sharps and the employer did not enter "privacy case" in the space normally used for the employee's name.

The description did not describe the cause of the wrist sprain.  b) On or about December 26, 2011, a recordable injury occurred when an employee strained their neck, shoulder and back, which resulted in job restrictions.  The description did not describe the cause of the strained neck, shoulder and back.

The description did not describe the cause of the shooting pain.  b) On or about May 1, 2013, a recordable injury occurred when an employee injured their right wrist, which resulted in treatment beyond first aid.  The description did not describe the cause of the right wrist injury.  c) On or about October 29, 2013, a recordable injury occurred when an employee pulled a muscle in lower back and buttock which resulted in job restrictions.  The description did not describe the cause of the pulled muscles.  d) On or about November 8, 2013, a recordable injury occurred when an employee sprained their right ankle, which resulted in treatment beyond first aid.  The description did not describe the cause of the ankle sprain.  e) On or about December 30, 2013, a recordable injury occurred when an employee injured their right shoulder and head, which resulted in job restrictions.  The description did not describe the cause of the shoulder and head injury.
