On or about 10/21/14, at the site of 157-26 9th Ave. Flushing, NY     Employees installing siding of the 2-story house were in danger of head injury from flying objects such as, but not limited to tools and siding materials.  Protective helmets were not provided.      ABATEMENT VERIFICATION IS NOT REQUIRED PURSUANT TO 29 CFR 1903.19

On or about 10/21/14, at the site of 157-26 9th Ave. Flushing, NY      Employees working on a pump jack scaffold were exposed to the hazard of falling approximately 16 ft. to the ground. The mid rails of the scaffolds were missed.    ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19
