OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:       A.) Paving/Road Milling Site  Parking Lot Situated Between Montana Chop House and The Movies off of 11185 Perry HighwayMeadville, PA: On or About October 20, 2014 and at times prior thereto, employees performing road milling and related work were potentially exposed to struck-by hazards from skid steers that were not equipped with functioning back-up alarms. The back-up alarm on the Case 1845C skid steer was not functioning.        B.) Paving/Road Milling Site  Parking Lot Situated Between Montana Chop House and The Movies off of 11185 Perry HighwayMeadville, PA: On or About October 20, 2014 and at times prior thereto, employees performing road milling and related work were potentially exposed to struck-by hazards from skid steers with obstructed rear views operating in reverse were not equipped with back-up alarms.  The newer Case 60 XT skid steer was not equipped with a back-up alarm.
