29 CFR 1910.1026(k)(3)(i): The employer did not ensure that the medical examination consisted of a medical and work history with emphasis on: Past, present, and anticipated future exposure to chromium (VI); any history of respiratory system dysfunction; any history of asthma, dermatitis, skin ulceration, or nasal septum perforation; and smoking status and history: (a) JK Hill & Associates Inc, at 7285 4th Street, Hill Air Force Base, UT: On and preceding 10/22/14, JK Hill & Associates Inc did not ensure that the medical examination consisted of a medical and work history with emphasis on: Past, present, and anticipated future exposure to chromium (VI); and any history of asthma, dermatitis, skin ulceration, or nasal septum perforation. One employee abrasively blasted aircraft in PEG 220D9 within Building 220. This process generated hexavalent chromium dust. On 10/22/14 the employee was exposed to hexavalent chromium at a concentration greater than the 8 hour Time Weighted Average (TWA) Action Level (AL) of 0.025 mg/m3. The employee was exposed to hexavalent chromium at a concentration of 0.005 mg/m3 as an 8 hour TWA. This is 2.0 times the AL. The employee performed this task more than 30 times per year. The employee's medical examination did not include a medical and work history with regards to dermatitis, skin ulceration, or nasal septum perforation.

One employee abrasively blasted aircraft in PEG 220D9 within Building 220. This process generated hexavalent chromium dust.
