29 CFR 1926.501(b)(1): 29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  LOCATION: 1 Brewster St, Randolph MA:   An employee was exposed to a fall of 9 feet to a lower level, while working on a unfinished loft, where fall protection had not been used.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  LOCATION: 1 Brewster St, Randolph MA:  The portable step ladder being used by employees to access the third floor loft, did not extend 3 feet above the loft's landing surface.
