Worksite located at 601 NE 70th Street, Gladstone, Missouri- Employees were exposed to overhead hazards from materials and employees working on the roof level above where the employees were working below without the use of any means of head protection.

Worksite located at 601 NE 70th Street, Gladstone, Missouri- Employees were exposed to a fall hazard in that employee were not wearing any means of fall protection while working in an aerial lift.
