29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  (a) Second floor top plate, new residential construction located at 3114 Staglin Drive, Powder Springs, GA 30127: Employee was exposed to eye injuries while operating a pneumatic Hitachi (Model - NR83A3) Strip Framing Nail Gun without eye protection. Employee was exposed to flying plastic debris and fasteners while operating a pneumatic tool at 120 PSI without safety glasses or goggles.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) Second floor top plate, new residential construction located at 3114 Staglin Drive, Powder Springs, GA 30127: Employee was exposed to falls of approximately 20-feet, 6-inches while working from the top plate installing a pre-fabricated gable truss without a fall protection system in place. The exposed employee was wearing a full body harness with no connection to a lanyard and life-line.
