On or about October 23, 2014, employees were exposed to fall hazards in that the employer did not ensure employees installing roof trusses on top of a building above 6 feet were protected from falling by a guardrail system, safety net system, or a personal fall arrest system.

Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws: On or about October 23, 2014, employees were exposed to electric hazards in that the employer did not ensure a strain relief was provided on a saw's flexible cord to prevent pull being directly transmitted to joints or terminal screws.
