29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about October 23, 2014, employees were exposed to fall hazards in that the employer did not ensure employees installing roof trusses on top of a building above 6 feet were protected from falling by a guardrail system, safety net system, or a personal fall arrest system.

29 CFR 1926.403(b)(1): Examination. The employer shall ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees.   On or about October 23, 2014, employees were exposed to electric hazards in that the employer did not ensure a flexible cord on a Makita saw did not have damaged inner and outer insulation.

29 CFR 1926.405(g)(2)(iv): Strain relief. Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws:   On or about October 23, 2014, employees were exposed to electric hazards in that the employer did not ensure a strain relief was provided on a saw's flexible cord to prevent pull being directly transmitted to joints or terminal screws.
