A feasible and acceptable abatement method to correct the hazard would be to:  1) Provide adequate training and instructions to employees to wear and utilize personal protective equipment, including properly tying off and properly securing anchorage points. 2) Train the employees in accordance with ANSI/TIA-1019-A Section 2.15 of Annex A.

This occurred on or about October 8, 2014.

The employees tied off to a vent pipe and a ladder which were not capable of or designed to support 5000 pounds per employee attachment. This occurred on or about October 8, 2014.
