29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. a. On or about October 22, 2014 -at the above addressed jobsite, an employee working in a GENIE S-60 telescopic boom aerial lift was not properly attached to the boom or basket by the use of a lanyard, thereby exposing the employee to the hazards associated with a fall of more than fifteen (15) feet.

29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable: a. On or about October 22, 2014 - at the above addressed jobsite, an employee using the Genie S-60 telescopic boom aerial lift to perform carpentry operation on the outside of the building was not authorized and or trained to recognize the hazards associated with the use of aerial lifts, thereby exposing the employee to the hazards associated with falls.
