29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: On or about October 31, 2014 a breaker box located in the area behind the fish tanks had two unused openings on the lower right side. Employees accessing the breaker box to reset breakers were exposed to the hazards of contacting energized electrical parts.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: On or about October 31, 2014 flexible cords supplying power to electronic ballasts for fluorescent lighting above the tanks were spliced to conductors on the ballast. Strain relief was not provided to prevent pull from being directly transmitted to joints or terminal screws. This condition exposed employees to the hazard of electrical shock.

29 CFR 1910.303(f)(2): Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident: On or about October 31, 2014, breakers inside the breaker box located behind the fish tanks were not marked to indicate the circuit or equipment protected by the breaker. This condition exposed employees to the hazard of no being able to locate the disconnecting means to equipment such as lights and pumps in the event of an emergency.
