29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.  Warehouse: The employer did not provide formal instruction and training to powered industrial vehicle operators that consisted of lectures, discussion, video tapes, written materials, tests, and practical training to ensure operators understand the safe operation of the vehicles and to prevent struck-by hazards.  No documentation or certification required for this item.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area.  Furniture Medic Department: Employees are required to work with chemicals such as, but not limited to spray paints, toners, lacquers, thinners, and glue without training on the associated safety and health hazards of the chemicals to prevent respiratory and skin irritation.  Documentation and certification is not required for this item.
