29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  On or about 10/29/14 located at 4735 Lowell Ct., Cumming, GA:  The employer did not perform frequent and regular inspections where employees were installing plywood on the roof without fall protection and no safety glasses. Employees were using a make shift scaffolds and were not using hard hats. Employees were not using safety glasses with the nail gun and the skill saw.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   On or about 10/29/14 located at 4735 Lowell Ct., Cumming, GA: Employees were framing a house with lumber, floor joist and scrap  2 x 4s  were observed falling to the ground as employees was working below tossing up scrap lumber to the 3rd floor, without hard hats exposing them to a struck by hazards.

On or about 10/29/14 located at 4735 Lowell Ct., Cumming, GA:  Employees were observed framing a house while using a pneumatic nail gun and the skill saw without safety glasses.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   On or about 10/29/14 located at 4735 Lowell Ct., Cumming, GA:  Employees were exposed to 24 foot falls and struck- by hazard, while preforming framing work from the make shift walking / working surface. The employer failed to install guard rails and it was not fully decked.

On or about 10/29/14 located at 4735 Lowell Ct., Cumming, GA:  The employer failed to provide personal fall protection for employees that were exposed to fall hazards of up to 40 feet and while framing a house under construction.
