OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to chemical burn, inhalation, and explosion hazards from the uncontrolled release of anhydrous ammonia.     a) In the engine room, on or about 11/19/14: The two compressors primarily used for the operation of the ammonia refrigeration system: A Frick rotary screw compressor, RDB 1778, and a Frick shell chamber compressor, 138152, were equipped with pressure relief valves which would vent inside the engine room.     b) In the engine room, on or about 11/19/14: 6 backup compressors inside the engine room were not equipped with any means of pressure relief.     c) In the engine room, on or about 11/19/14: A pressure vessel, Intercooler #2 was equipped with a pressure relief valve which would vent inside the engine room.     d) In the engine room, on or about 11/19/14: The only means of ventilation for the engine room was a wall fan which did not continually operate.    e) In the engine room, on or about 11/19/14: The engine room was not sealed to prevent escape of anhydrous ammonia to other areas of the facility. Entry into the engine room was though an open walkway, there was no door.     f) In the engine room, on or about 11/19/14: The engine room was not equipped with an ammonia detector/alarm system to notify personnel if a release of ammonia occurred.    g) Next to warehouse rooms 9 and 10, on or about 10/29/14: An insulated pressure vessel and associated piping was covered in ice buildup to the point that valves were inaccessible.      Abatement certification and documentation/evidence of abatement is required by 29 CFR Part 1903.
