29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:    a) On or about 10/30/14 at residential roof replacement at 9067 Otto - East Otto Road, Otto, New York; employees were using a ladder jack scaffold while tearing off shingles. Employee were not using personal fall arrest system.    NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a) On or about 10/30/14 at residential roof replacement at 9067 Otto - East Otto Road, Otto, New York; employees were replacing a roof on a house that had a roof pitch of 12 in 12 with eave heights of approximately 10 and 18 feet. There was no fall protection in place.    NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a) On or about 10/30/14 at residential roof replacement at 9067 Otto - East Otto Road, Otto, New York; employees were using an extension ladder to access the work area on the roof. The ladder extended 1 foot above the landing surface.    NO ABATEMENT CERTIFICATION REQUIRED
