29 CFR 1926.651(k)(2): Where the competent person finds evidence of a situation that could result in a possible cave-in, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions, exposed employees shall be removed from the hazardous area until the necessary precautions have been taken to ensure their safety:   (a) On or about 10/28/2014 - 30018 Hardiman Rd, Madison, AL, employees were working in a trench  without adequate protection and in close proximity to a 4 inch gas line.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c):   (a) On or about 10/28/2014 - 30018 Hardiman Rd, Madison, AL, one employee was spreading gravel in the bottom of a trench outside the confines of the trench box at 13 feet below the ground.   (b) On or about 10/28/2014 - 30018 Hardiman Rd, Madison, AL, one employee was spreading gravel in the bottom of a trench box that extended 8 feet above the ground. The area above the trench box had 5 feet of exposed ground that could have caved-in.
