On 10/03/2014, workers from Enviro Tech Construction were repairing a roof of a commercial building located at 3833 McClay Road, St. Peters, MO.  The employees were not wearing eye protection while nailing shingles to the roof.

29 CFR 1926.453(b)(2)(v):   A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift.  On 10/03/2014, workers from Enviro Tech Construction were repairing a roof of a commercial building located at 3833 McClay Road, St. Peters, MO.  An employee was installing shingles from an aerial lift, model # E400AJP Narrow.  The employee was not wearing a body belt with a lanyard attached to the booms or basket.

Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  On 10/22/2014, employees from Enviro Tech Construction, LLC were repairing a roof of a commercial building located at 3833 McClay Road, Saint Peters, MO.  One employee was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  The employee was wearing a harness, but was not tied off.
