29 CFR 1926.451(g)(1): The employer did not ensure that each employee on a scaffold more than 10 feet (3.1 m) above a lower level was protected from falling to that lower level.   This violation occurred on October 28, 2014, where at least one (1) employee working on an elevated work platform was not protected from falling more than 20 feet to the concrete below by an adequate guardrail system.

29 CFR 1926.454(a): The employer did not ensure that each employee who performs work while on a scaffold was trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.   This violation occurred on October 28, 2014, where at least one (1) employee working on an aerial lift without fall protection was exposed to falls of more than 25 feet to the concrete below.
